About Us

A Hand To Hold is made up of licensed professionals who specialize in senior care including care coordination and management, advocacy, and problem-solving.  The owner has been a geriatric nurse for over 28 years and in addition to having certification in case management and gerontology, holds credentials as a Certified Senior Advisor, Aging Life Care Manager ™ (the new term for a Geriatric Care Manager), and Certified Dementia Practitioner. Our geriatric care management team has experience in all facets of geriatric health including acute, long-term, home health, and hospice care. We are licensed, insured, and have had extensive background checks performed.  Our goal is to be a trusted resource and to create relationships with our senior clients and their families that provide them with skilled, knowledgeable and caring support.

Carlsbad, Encinitas, Del Mar, Oceanside, Vista, San Marcos, and Escondido are our current areas of service. We are also able to assist those living in other areas of San Diego County depending on the situation, please call us for details.
