At A Hand To Hold, we put our heart into our work as Aging Life Care Managers ™ (the new term for geriatric care managers) and care coordinators in San Diego and Orange Counties. This means that we spend time getting to know our seniors; their histories, strengths, wishes, and challenges. We listen to, partner with, and guide, our geriatric care management clients using our expertise and years of experience in the senior care field.

Our goal as geriatric care managers is to support both our clients and their families; we strive to decrease everyone’s stress and anxiety levels while providing practical solutions to aging challenges. We focus on our client’s wellbeing as a whole, including their desire to be independent, and together we look at available options and co-create a personalized care plan that allows for optimal aging.
Our geriatric care managers are based out of Carlsbad but are able to assist seniors throughout San Diego and Orange Counties.
How We Can Bring You Peace of Mind
Our geriatric care managers provide one-time care consultations, family coaching packages, and transitional assistance which helps seniors to transfer safely from the hospital or skilled nursing setting back home. We also assist seniors with ongoing care management where we act as “step-in daughters”, coordinating and overseeing all aspects of a client’s health and wellbeing.
In addition, we offer various concierge in-home support services including personalized engagement activities that are tailored to each person’s unique needs for socializing with the additional goal of enhancing brain health as well as joy and happiness levels.

The “Is My Loved One Safe?” Package
Perfect for families who want to know if their loved one is safe in their current living situation ( in their home or senior care community settings). Safety assessments can be done when a senior begins to decline due to age, to prepare for aging-in-place in the future, or if a major change of condition has occurred such as recent surgery. We look holistically at the whole situation including the senior’s current health status, social support, the home setup itself, current challenges, medications, and more.
This package includes a discovery call, a home assessment by our geriatric care manager whose years of experience in the field of gerontology can help to pinpoint potential dangers, a comprehensive medical and social history review, a personalized action plan that can be put into place immediately, and a follow-up support and coaching call.
